Define Approved Patient Scenario

A patient is a 49-year-old man who has complained of suffering from progressive headaches for the last month. The medication was provided to relieve the headache. He was directed to the emergency room with the following symptoms: vomiting, nausea, and visual disturbance. Records indicate that the patient had initially been diagnosed with hypertension (HTN) hereditary; Beta-blockers were prescribed. Imperatively, he had not taken any medication within the last 12 months for reasons best known to him. The patient smokes cigarettes and occasionally engages in alcohol consumption, but denies using drugs for recreational purposes. Imperatively, his smoking habit is dangerous as it can alleviate his HTN condition to extreme levels (Akbarpour et al., 2018). The patient’s blood pressure was captured at 180/110, with his brain’s MRI manifesting scattered lesions that intensified around the posterior and occipital lobes.

It is also evident from the history that the patient is a college-level graduate and eager to learn ways to modify the lifestyle to maintain a healthy lifestyle. The eagerness to learn would be beneficial as the patient will be taught to use the mHealth app to remind him to have a healthy lifestyle and maintain his blood pressure. With concerns placed to improve his condition and remind him to take his medicine on time, a mHealth application is needed. The application in concern for this assignment was CareClinic.

Identify Teaching Areas

Technology can be used to achieve such a cause by giving the patient a mobile application called a CareClinic app that can monitor the progress of healing, the medication’s effectiveness, and provide guidelines on diet, exercise, medication, and other things. The information on CareClinic app (CareClinic Tracker & Reminder, 2020) are:

· CareClinic app is a symptom and medication tracker and a health journal that helps measure, learn, and improve your health.

· It is available for Android and iPhone.

· Download on Android from google play store and iPhone from Appstore.

· Benefits of the CareClinic app

· It helps to create your own care plan where you can track your daily symptoms.

· You can schedule your medication time, and the app will remind you to take medicine. 

· Manage acute, chronic, and preventive medical care in one app.

· Record and manage medications

· Manage health stats (i.e., blood pressure, weight, cholesterol)

· It’s essential to Logout after each session in case your phone becomes lost or stolen.

· Put identification, authentication, and authorization measures in place (Wodehouse, 2019).

. Make sure your phone has a password that only you and trusted individuals may access.

· Create a strong password to access ‘CareClinic app’ (Uppercase, lowercase, symbols, or numbers).

· Another safety precaution that the patient needs to take is to ensure that one adheres to the application’s prescriptions and recommendations after feeding it with the correct information

· Interpretation:

The ways to interprete and act on the information that is provided by the mHealth app for this project are:

· It is easy to navigate the application and register a new account.

· After the registration, the user can add the existing health problem that will help connect to the symptoms.

· Users can go to “my care plan” tab in the app to add the medicine and supplements they are taking, nutritional plan, activity plan, symptom check, and measurement check. This tab allows the patient to add and delete different checks per the patient’s condition. By entering data of the patient’s blood pressure on the measurements, one can keep track of maintaining the blood pressure within normal limits. For instance, if the blood pressure is 160/90, and you have been in 120-130/80-90, it determines certain changes and might need medical help. This will help him to imrove his health status. Making the correct interpretation of medical terms and health circumstances is a critical point of the teaching area.

· The “check in” tab in the app reminds you if you have taken your medication.

· You can also turn on the notification from setting to notify the reminder to take medicine or for daily check-in. This will help a patient to know about his health stats daily.

Determine and Evaluate Success (3)

· I would ask Mr. NB to demonstrate back how to use the app without any assistance. This will reveal the understanding of the usage of the app by the patient. The teach-back method of instruction is a great way to evaluate patients’ learning outcomes and the success of teaching (Farris, 2015). 

· I would ask him about his understanding of hypertension and the importance of taking medication. This will give me a clear picture of what more information is necessary for the patient and add data in his care plan in the CareClinic app.

· I would have Mr. NB check his blood pressure, blood glucose, weight, and waist measurements regularly, documenting them in the application. Entering his findings in the application will show that he successfully knows how to input his results in the application and manage his health by seeing his progress.

Mr. NB, a 49 years old moderately obese man, came to the emergency department with a month’s history of progressive headache presented to the emergency department with nausea, vomiting, and visual disturbance. Headache was unrelieved by medication. It is noted that he was previously diagnosed with hypertension (HTN) and was prescribed with Beta-blockers but has not taken medicine for a year now for an unspecified reason. He is an active tobacco smoker and drinks alcohol occasionally. Denies of recreational drug use. He has family history of HTN to both father and mother. On arrival, his blood pressure was noted to be 180/110. MRI of the brain demonstrated scattered lesions with increased intensity in the occipital and posterior parietal lobes. But no acute CVA noted. The clinical presentation is suggestive of hypertensive emergency and hypertensive encephalopathy. He has a college-level education. After knowing about his current condition and future possibility of having stroke, the patient is more willing to learn about the ways to control his blood pressure and bring change in lifestyle.

As per the information obtained from Mr. NB’s assessment, it was revealed that he needs to modify his lifestyle, and proper education is necessary to the patient. Education will be more focused on lifestyle modification that includes encouraging exercise and reducing weight, dietary changes, and consultations with a dietician/nutritionist. It will be beneficial if the patient is taught about self-monitoring skills, provide information on their target blood pressure, and tell them about its importance to maintaining the targeted blood pressure. We will provide education on the medication regimen he will be in as the patient had previously stopped taking medication. He will be provided with educational materials like pamphlets about the disease condition, sample diet like low salt diet and cardiac diet, and encouragement to light exercise with walking. Mr. NB will also be discussed in detail about the medication regimens that include when and how to take medicine, how the drug works, and the adverse effect. The addition of hydrochlorothiazide 25mg daily three times a day and metoprolol 50mg twice a day.
