1. Must have Home Assessment Assignment Paper completed and each question must be answer in paragraph form of more than 3 sentences. (File Attached).
2. Overall report paper double space and type in roman numeral front. the overall paper must consist of everything answered in step 1.
3.Also, the paper must have 3 resources and references( For each resources you must describe how each resource will benefit the client and what type of resource it is.)
4.The paper must include a ethical dilemma( example given was patient want take medication/ get medicine because of having to pay for rent.

Home Assessment Guidelines and Objectives
OBJECTIVES: The associate degree nursing student will demonstrate a holistic
understanding of a home assessment by:
1. Conducting an interview of an individual regarding physical, spiritual, cultural, social, and financial needs/issues.
2. Conducting a home assessment for potential safety risks.
3. Analyzing the assessment findings and providing evidence based solutions.
4. Discussing available community resources that can benefit the individual.
GUIDELINES:
1. Select an individual that is 55 years old or older who is disabled, or who is retired and 65 years old or older.
2. Describe the individual in detail. It must be descriptive of posture, appearance, articulation, mood and affect.
3. Describe individual’s neighborhood, the outside home environment, the inside of the home, and any actual or potential safety hazards.
4. Interview the individual regarding physical, spiritual, cultural, social, and financial needs/issues.
5. Discuss at least three community resources that benefit or could benefit the individual.
6. Discuss at least one ethical dilemma faced by the individual and solutions to the issue.
7. Provide evidence based resources to support your findings.
8. Use correct, grammar, and syntax.

GRADING CRITERIA:
S = Satisfactory – consistently meets objective and does the job well. Student demonstrated knowledge and preparation in performing all aspects of the behavior.
U = Unsatisfactory – inconsistently meets objective, requires maximum assistance to meet objective, or fails to meet objective. Clinical behavior and / or work is either unsafe, inconsistent, or inadequate in meeting all aspects of the performance described in the behavior.
I = Incomplete – partial completion of objective. Actions/observations were satisfactory in areas completed yet did not fulfill entire requirement.
N = No Opportunity/Beyond student’s ability to complete: – No opportunity for the objective to be implemented (or) the objective required advanced knowledge/skills not previously acquired in the program.
DEFINITIONS
With Assistance:
1. With guidance from the instructor, the student is able to utilize suggestions, explanations, and directions to meet objective.
Maximum Assistance:
1. Is unable to utilize suggestions, explanations, directions, and/or supervision to function safely.
2. Is unable to transfer knowledge and guidance from one situation to another similar situation.
CALCULATION OF YOUR GRADE IS DETERMINED BY:
1. Subtracting the “N’s” from the total number of objectives possible for experience = your total possible satisfactory points.
2. Subtracting the “U’s” from your total possible satisfactory points
3. Subtracting the “U’s” obtained from “I’s” (2 I’s = 1U) from your total possible satisfactory points
4. Dividing your total satisfactory points by the total possible satisfactory points (obtained in #1) = your clinical grade.
FOR EXAMPLE:

# of “U” evaluations – 10
# of U’s obtained from I’s -5
(10 I’s = 5 U’s)
