Effective Care in End of Life Management

Palliative care is delivered to patients and their families when they are at their most helpless and vulnerable situations. Undoubtedly, palliative care is a perfect fit APRNs since they love the challenge of working with difficult and complex situations in addition to enjoy building close relationships with their patients and families. This type of care focuses on providing relief from physical symptoms and stress of serious or terminal illnesses (e.g., cancer), in addition to address the whole person. Likewise, palliative practice offers the opportunity to work thoroughly with an interdisciplinary team, including psychologists, nurses, physicians, social work, dietitians, pharmacists and volunteers, all functioning to treat challenging symptoms such as pain, depression, fatigue, constipation and a number of other symptoms associated with chronic medical conditions and mental health (Schroeder & Lorenz, 2017). 

APRNs play a unique role in palliative care. As APRN typically implement a holistic approach to address the physical, emotional, spiritual and social aspects of the patient’s and family’s needs. At present, the Institute of Medicine (IOM) recommends that care decisions be evidence-based to effectively and efficiently care for patients and provide effective care in the end of life. This type of approach has been shown to improve patient outcomes and reduce health care costs throughout the lifespan, particularly at the end of life. Accordingly, a palliative care is regarded an evidence-based intervention and result in a higher quality of life and increased dignity, emotional and spiritual support, and patient and family well-being. Evidence suggests that patients receiving palliative and hospice care live longer when compared with similar patients who do not receive those services. At present, hospice services extend palliative care principles to the end of life (Dobbins, 2016). 

In summary, multiple studies have emphasized the importance of meaningful conversations on an individual level in which key aspects related to patients’ values, goals, and preferences of care are discussed. Typically, most patients are not aware of the choices they have at the end of their lives. Therefore, APRNs should use the information and materials available to inform and advise their patients of possible options. Definitively, APRNs are recognized as skilled clinicians who interpret patient responses to diseases and treatments, advocate for holistic and effective care, facilitate relationships with other care providers, and provide bio-psychosocial care. Nowadays, APRNs serve as primary care hospitalists, administrators and health care providers. In addition, they function as researchers, educators, and advocates, and researchers. APRNs are perfectly positioned to improve the quality and availability of medical and social services for patients and their families, enhancing quality of life through the end of life (Dobbins, 2016).

           Palliative nurse practitioners are advanced practice nurses who treat patients with life threatening illnesses. They assess patient needs and diagnose health challenges. Palliative care nurse practitioners are skilled clinicians who are knowledgeable about a wide range of medical conditions. They provide emotional support to patients and family members facing devastating diagnoses.

It is important for nurses to implement communication strategies when working in teams as it helps them define the scope of public health nursing practice by the type of intervention and the level of practice (American Public Health Association, 2016). As our lesson notes, “both oral and written communications are important in nursing and healthcare” and this is also critical in teamwork assignments. Some communication strategies that aid in the collaborative process are to set clear and cohesive expectations, engage in open and honest conversations, and follow-up through meetings, written material, or electronic communication.

           With an aging population and healthcare reform, palliative care is an essential component of the work of nurse practitioners. Inpatient and community-based palliative care programs are helping health systems, ACOs, physician practices, and skilled nursing facilities provide care that enhances patient outcomes, decreases hospitalizations, and improves patient satisfaction (Johnson & Gray, 2013).

           Palliative and end-of-life care programs help improve care quality throughout the course of a patient’s illness. Patients that utilize palliative and end-of-life care tend to be more satisfied with their overall care and communication with providers, and they are less likely to end up in intensive care units and emergency departments (Johnson & Gray, 2013).

           Collaboration is taking action to reach a common goal (Fawcett, 2010). It commits two or more persons or organizations to achieve a common goal through engagement, partnerships, and advocacy. For example, at the University of Miami Health, the nurses in my unit collaborated with the Florida Department of Health and other local agencies to plan a program for children experiencing developmental problems. Processes such as problem setting, direction setting, and structuring were addressed as some of the communication strategies used to accomplish the project’s goals.

End of life management refers to the health care given to individuals with chronic conditions that are advanced and incurable. End of life management is a demanding task that requires effective decision making that includes medical intervention and palliative care. On the other hand, Advanced Practice Registered Nurse (APRN) provide primary care to the public, and they include clinical nurse specialists, nurse midwives, and nurse anesthetists. APRNs are responsible for diagnosing illness, providing care for individuals with chronic disease, and updating the public on healthy living. Also, APRNs continuously educate themselves to keep up with changing methodologies and technology in their jurisdiction. For APRNS to provide adequate care in end of life management, they are continually engaged in professional education, effective communication, and pain management programs.

 

Advanced Practice Registered Nurses (APRN) are required to have mastery of life skills and how to perfectly manage different situations at work. They are often tasked with the role of diagnosing illness, providing care for individuals with chronic disease, and updating the public on healthy living. Also, APRNs provide adequate care in end of life management, through their overall practice within the medial realm (Tse et al., 2020).

 
 

Education

APRNs can perfect their effective care through enrolling in educational programs that focus on palliative care. There are different educational programs that offer health practitioners knowledge on palliative care. Educational programs help improve engagements between the patient and the nurse improving the patients’ quality of life by communicating effectively to them and their loved ones (Koh et al., 2017). Effective communication is also significant to APRNs when breaking bad news to patients, dealing with temper problems, and empathetic communication. Effective communication also enables APRNs to be mindful of patient’s point of view, thus allowing them to counsel the patients concerning their various needs.

Research

Research plays an important role in taking care of patients with terminal illnesses. Since health professionals spend more time with patients in pain, it is essential that APRNs enroll in in depth research programs. (Jomy & Hapidou, 2020). The practice helps equip APRNS with the necessary knowledge and skills to manage end of life . These include dealing with psychosocial pain issues, pharmacology, pain assessment, and ethical issues (Tse et al., 2020). In sum, at the end of the curriculum, APRNs gain the ability to manage patients’ pain and implement roles relating to staff interfacing.

Administration

Administration is a key part of health. It offers nurses the ability to evaluate and assess the needs of patients. Besides, it also ensures that nurses are competent in taking care of patients’ chronic pains. Besides, through collaborative engagements with nurses ,the administration increases APRNs’ power scope, wages, and working conditions. Therefore, the ability to provide effective end-of-life management improves the quality of patients’ lives and advances APRNs’ professionalism.
